The Trent Hills Family Health Team is made up of nine physicians who practice family medicine full time. One additional member of the Team combines GP/Anaesthesia work with a part-time practice. Three of these nine physicians (along with two other family physicians at solo locations) formed the Northumberland Family Health Network (NFHN) in 2002. This FHN, one of the first in Ontario, has become the core of the Trent Hills Family Health Team (THFHT) which was implemented in early 2006.
The main site is located on the upper floor of the Campbellford Memorial Health Centre. The Team includes four nurse practitioners, two social workers, registered nurses, a dietician, pharmacist and other allied health professionals and administrative staff . The Team currently delivers primary care services to over 16,000 patients.
The THFHT is continually developing and implementing various programs, services and education for patients. All programs and services are delivered in an interdisciplinary environment using a collaborative care approach. Current programs and services include: Foot Care, Anticoagulation, Appointment Wait Time Reduction, Geriatric Mental Health Care, Weight Loss Counselling and overall health promotion.
