CMH continues to work diligently to improve the quality of life for heart patients through our Cardiac Education Program.
This out-patient program promotes lifestyle changes for individuals who are at risk or have recently developed cardiovascular disease. The program provides education delivered by a multidisciplinary team, as well as follow-up and support for individuals and their families.
Our goal is to promote primary prevention and improve the quality of life for heart patients by providing education about risk factors and lifestyle changes. Cardiac Group education sessions are offered every month Thursday and Friday mornings from 9 a.m. to 12 noon (Check timing). Handouts, literature and an individual follow up visit is included. CMH is also a partner in the Haliburton, Victoria, Northumberland Heart Health Project. As partners, we are committed to working together to plan, implement and evaluate heart health initiatives that address the goals and objectives of the Heart Health Program.
