Information We Collect
Customer Information:
In order to purchase a product or service, you may be required to give us contact information (e.g. name, email address), unique identifiers (e.g. password), and demographic information (e.g. postal code). IP address:
We collect your Internet Protocol (IP) address each time you visit our web site. The IP address is a unique identifier assigned to your computer each time you connect to the internet. The IP address is used for site traffic statistical reporting and analysis, to help diagnose problems with our server, and to help protect our customers and ourselves from fraud.
Cookies:
Our web site uses cookies to keep temporarily stored information during a multi-page process, such as when placing an order. A cookie is a small amount of data that is sent to your browser from our server and stored on your computer's hard drive. You have the choice to accept all cookies, or be notified when a cookie is set, or to reject all cookies, by setting the appropriate options in your browser. We collect personal health information about you directly from you or from the person acting on your behalf. The personal health information that we collect may include, for example, your name, date of birth, address, health history, records of your visits to Campbellford Memorial Hospital and the care that you received during those visits. Occasionally, we collect personal health information about you from other sources if we have obtained your consent to do so or if the law permits.

- We may legally disclose just your contact information with the CMH Foundation to help raise funds. Please contact the CMH Foundation to opt out of fundraising programs
