The Trent Hills Family Health Team (THFHT) is an interdisciplinary primary health care model staffed by 7 physicians (FHO model). Provincial funding is provided for our four nurse practitioners, three Mental Health Workers, registered nurses, registered practical nurses, an Executive Director and support staff. Our main site is located at 119 Isabella Street in Campbellford, Ontario, with three additional sites in Trent Hills (Warkworth and Hastings), and Colborne. The THFHT provides a full range of primary care services and programs. Approximately 12,000 patients complete our roster in the Municipality of Trent Hills and adjacent areas.
The THFHT was one of the first FHTs approved by the Ontario MoHLTC in 2005. Incorporated in 2006, the THFHT is governed by a Board of Directors, the membership of which currently includes three physicians and four community members. The THFHT has an annual operating budget of approximately $2 million.
In April of 2022 we worked the Ministry of Health and introduced our new enhanced FHT model as our Nurse Practitioners are now in the process of registering their own practices. This patient base is from the practices of two physicians who retired and one who went on to a pursue palliative care.
We have switched to a hybrid model and are still offering some virtual (telephone) appointments and virtual mental health groups since the pandemic started in 2020. We offer extended hours to our patients Monday through Thursday and Saturday mornings. We are also in the process of revisiting our patient programs and services to best align with our patient population needs and our funders (Ministry of Health).
Current patient programs offered include: Diabetes Management Program (in collaboration with CMH), Anticoagulation therapy, Smoking Cessation (in collaboration with the Ottawa Heart Institute), Virtual Mental Wellness Groups, Well Baby Program, Nutrition Program (FUEL). Our PATH (Palliative and Therapeutic Harmonization Model) is a proven intervention that improves the quality of care for people living with dementia.
Services include: Phlebotomy (venipuncture), Cognitive Screening, Rapid Access appointments, Cervical Screening, Colorectal Screening, Breast Screening all in collaboration with Cancer Care Ontario, Flu Immunization, other Immunizations, and Electrocardiogram service (ECG).
The THFHT has a close partnership with Campbellford Memorial Hospital and The Bridge Hospice, Warkworth ON. We are also part of Ontario Health Team- Northumberland since inception in 2019.
