Release of Your Personal Health Information (Health Records)

Campbellford Memorial Hospital is committed to protecting patient privacy and places the highest value on maintaining the confidentiality and security of personal health information (PHI). We’re proud to partner with six other hospital organizations in the Central East region to provide patients with a single, unified digital health record through the Epic Clinical Information System.

If you would like a copy of your health records, or would like to share your records with someone other than yourself, please fill out the appropriate form below and return to healthrec@cmh.ca
