What Clients can Expect

Client Assessment

Clients will receive an assessment at our clinic.  Two members of the team will meet with the senior and their caregiver/family where recommendations and care plans will be created with the client, caregiver and primary care provider.

It is important to be prepared for the appointment.  Please ensure that you have available with you at the appointment:

• walking aids
• eye glasses
• hearing aids
• all medications, including over-the counter products (please bring all medication containers, bottles, eye drops, vitamins, creams, etc.). Keep all original labels on the containers or packages
• a light lunch/snack – the assessment may take up to 2 hours
