Trent Hills Family Health Team

The Trent Hills Family Health Team (THFHT) is an interdisciplinary primary health care model staffed by 7 physicians (FHO model). Provincial funding is provided for Nurse Practitioners, Mental Health Counsellors, Registered Nurses, Registered Practical Nurses, an Executive Director and support staff. Our main site is located at 119 Isabella Street in Campbellford, Ontario, with two additional sites in Trent Hills (Warkworth and Hastings). The THFHT provides a full range of primary care services and programs. Approximately 13,000 patients complete our roster in the Municipality of Trent Hills and adjacent areas.

The THFHT was one of the first FHTs approved by the Ministry of Health and Long Term Care in 2005. Incorporated in May of 2006, the THFHT is governed by a Board of Directors, the membership of which currently includes two physicians and four community members. The THFHT has an annual operating budget of approximately $2.4 million.

In April of 2022 we worked with the Ministry of Health and introduced our new enhanced FHT model as our Nurse Practitioners now have their own practices. This patient base is comprised of patients from the practices of the three physicians who left our FHO/FHT in 2022, along with many newcomers to our area.

Since the pandemic, we still run a hybrid model by offering some virtual (telephone) appointments. Our team offers some extended hours to patients Monday through Thursday evenings and Saturday mornings. Our programs and services have been revised to best align with our patient population needs and our funder's (Ministry of Health/Ontario Health) expectations.

Current patient programs offered include: Women and Children’s Health, Adult Screening and Prevention, Older Adult Primary Care, Cancer Survivorship, Healthy Heart, Lifestyle and Nutrition Counselling, Mental Health Counselling and referrals. Services include: Rapid Access/same day appointments, Flu Immunization and other immunizations, nursing episodic care, some venipuncture, in- person and or/virtual care appointments.

The THFHT has a close partnership with Campbellford Memorial Hospital and The Bridge Hospice, Warkworth ON. We are also part of the Ontario Health Team - Northumberland (OHTN) https://www.ohtnorthumberland.ca since inception in 2019. 

Our Executive Director, a Nurse Practitioner and our Board Chair sit on various committees of the OHTN.
